Despeje o leite e vá mexendo até a tapioca absorver todo o líquido. Ferva o 1 litro restante do leite e despeje aos poucos a mistura até obter uma consistência cremosa e mole. Unte uma forma com óleo de coco e coco ralado e despeje a mistura. Cubra com papel alumínio e leve a geladeira por 3 horas. Cozinhe o macarrão em água com sal por 10 minutos, escorra e reserve. Lave o manjericão, seque e tire as folhas, reserve. Em uma panela refogue o alho no azeite em fogo baixo e espere esfriar, junte no liquidificador o azeite com alho, o manjericão, as nozes e o parmesão. Bata bem até formar o pesto. Misture o pesto a massa.<br />
